Online apps now account for the majority of our new library cards. We issue them a card number and email that information, along with their 60-day expiration date back to them almost immediately (within about 24-48 hours). In person, we do not require them to fill out separate forms for each child, or any form at all actually. We never had any need to refer back to the paper forms and were just recycling them, so we only use them to hand out to a child who is at the library without a parent or someone who just wants to fill it out at home. For children, we use the parent's ID and trust them on the name of their child and the child's birthdate. As for the legality of what we do, I dunno.
